The applicant/claimant/respondent above named begs to state as under:—

1. That the aforesaid claim petition/arbitration dispute was referred to the Learned Arbitrator/Arbitral Tribunal in pursuance of the arbitration agreement contained in clause 125 of the Contract Agreement executed between the parties.

2. That the Learned Arbitrator entered into reference on ________
